What is It?

A Social Needs Assessment is a structured approach to identifying the social and environmental factors that influence a veteran’s health and well-being — such as housing, food access, employment, transportation, and social connection.

These social drivers of health can significantly affect risk for mental health challenges, treatment engagement, and suicide. Assessing these needs gives organizations the insights they need to:

  • Understand veterans’ full context
  • Connect them to the right services
  • Address barriers before they escalate

One key resource for conducting Social Needs Assessments is MAP Reach — a platform developed to support Face the Fight coalition members in strengthening veteran suicide prevention efforts.

MAP Reach: The Platform

Launched in 2024, MAP Reach (Measure, Assess, Prioritize) is a data-driven platform that equips organizations with tools to assess and respond to veterans’ needs. One of those tools is VSNI (Veteran Social Needs Index), which supports population-level understanding of social risk.

MAP Reach includes:

  • VSNI – A geospatial mapping tool that visualizes veteran-specific social drivers of health at the ZIP-code level
  • sDNA – A mobile-friendly, customizable assessment delivered directly to veterans via text or email
  • MCL & Alerts – AI-powered message analysis and real-time alerts for crisis indicators
  • Dashboards – Visualizations of veteran needs, trends, and outcomes at individual and population levels
